29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Eastward and Southward Facing Townhouse Buildings under Construction at the Southeast Corner of Admiral's Quay Drive, Mechanicsburg, Pennsylvania - Employees used pneumatic nailers to install wood framing, including roof sheathing and floor joists, and the employees doing the nailing and employees standing below and assisting did not wear hard hats, exposing the employees to a struck-by hazard from ricocheting nails, on or about August 30, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Eastward and Southward Facing Townhouse Buildings under Construction at the Southeast Corner of Admiral's Quay Drive, Mechanicsburg, Pennsylvania - Employees used pneumatic nailers to install wood framing, including roof sheathing and floor joists, and the employer did not provide the employees with eye protection equipment, exposing the employees to a struck-by hazard from ricocheting nails, on or about August 30, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  (a) Eastward Facing Townhouse Building under Construction at the Southeast Corner of Admiral's Quay Drive, Mechanicsburg, Pennsylvania - An employee installed roof sheathing on the three-story townhouse building under construction.  This employee worked three storied above the ground and did not use conventional fall protection (i.e., guardrail systems, safety net systems, personal fall arrest systems) or an alternative fall protection measure under another provision of paragraph 1926.501(b).  The employer also did not demonstrate that it was infeasible or that it created a greater hazard to use conventional fall protection and did not develop and implement a fall protection plan, exposing the employee to the hazard of falling, on or about August 30, 2018.  (b) Eastward Facing Townhouse Building under Construction at the Southeast Corner of Admiral's Quay Drive, Mechanicsburg, Pennsylvania - An employee worked inside the building on the second floor at window and door openings at the rear of this building under construction.  This employee did not use conventional fall protection (i.e., guardrail systems, safety net systems, personal fall arrest systems) or an alternative fall protection measure under another provision of paragraph 1926.501(b).  The employer also did not demonstrate that it was infeasible or that it created a greater hazard to use conventional fall protection and did not develop and implement a fall protection plan, exposing the employee to the hazard of falling, on or about August 30, 2018.  (c) Southward Facing Townhouse Building under Construction at the Southeast Corner of Admiral's Quay Drive, Mechanicsburg, Pennsylvania - Employees worked from top plates and already installed floor joists while continuing to install wooden floor joists for the third story of the townhouse building under construction.  The employees worked at the top of the second story above the ground and did not use conventional fall protection (i.e., guardrail systems, safety net systems, personal fall arrest systems) or an alternative fall protection measure under another provision of paragraph 1926.501(b).  The employer also did not demonstrate that it was infeasible or that it created a greater hazard to use conventional fall protection and did not develop and implement a fall protection plan, exposing the employees to the hazard of falling, on or about August 30, 2018.  (d) Southward Facing Townhouse Building under Construction at the Southeast Corner of Admiral's Quay Drive, Mechanicsburg, Pennsylvania - Two employees worked inside the building on the second floor at window and door openings of this building under construction.  These employees did not use conventional fall protection (i.e., guardrail systems, safety net systems, personal fall arrest systems) or an alternative fall protection measure under another provision of paragraph 1926.501(b).
