29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a) Worksite:  On or about 8/31/2018, employees were not protected from falling while working on a roof approximately twenty-one feet above a lower level.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:  (a) Worksite:  On or about 8/31/2018, the employer did not ensure that he and his employee had separate anchorage points to support them as part of a fall arrest system. Further, the employer only used four fasteners instead of the required six to install the hinged roof anchor being used as part of the fall arrest system.
