29 CFR 1904.32(b)(2)(ii): The annual summary did not include one or more of the following elements: the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log.  a) 633 Force Support Squadron (FSS): On or about 09/05/2018, the employer combined the recordable injuries and illnesses, number of employees and total hours worked from FSS with the establishments at Joint Base Langley-Eustis on the OSHA 300A Summary log or equivalent form.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees to OSHA within twenty-four (24) hours.  a)    633 Force Support Squadron (FSS): On or about 08/13/2018, an incident occurred at Fort Eustis in Building 843 that resulted in an employee hospitalized and released on 08/15/2018. The employer did not report the incident to OSHA until August 17, 2018.    No abatement certification or documentation required.

29 CFR 1904.40(a): The employer did not provide an authorized government representative copies of requested records kept under Part 1904 within four (4) business hours:    a)    633 Force Support Squadron (FSS): On or about 09/04/2018, the employer failed to provide copies of OSHA Forms 300 or their equivalents, which were requested for calendar years 2015, 2016, 2017, and 2018 to date.  No abatement certification or documentation required.
