Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) Jobsite - On or about August 29, 2018 employees installing ridge caps on a 4/12 roof were not protected from falling.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. (a) Jobsite - On or about August 29, 2018 an extension ladder being used by employees was not extended at least 3 feet above the landing of the 4/12 pitch roof edge. The roof edge was 45 feet above ground.
