29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a) 2106 Bernays Drive, York, Pennsylvania - The employer did not have a competent person involved in the job site in that none of the employees who worked on the site on September 4, 2018 inspected the job site, identified predictable fall hazards, and took immediate actions to correct the hazards. As a result, employees did residential roofing work on a two-story single family home without using any fall protection, exposing the employees to the hazard of falling to a lower level, on or about September 4, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) Roof - Employees worked re-roofing the two-story single-family home. The employees did not use any fall protection, exposing them to the hazard of falling from the roof to the ground below, on or about September 4, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
