29 CFR 1910.28(b)(11)(ii):  The employer did not ensure that each flight of stairs having at least three (3) treads and at least four (4) risers was equipped with a stair rail system and handrails per Table D-2:    At 185 West Outer Road, Valley Park, MO, specifically at the northeast corner of the repair shop, the concrete flight of stairs leading out to the yard was equipped with a single stair rail only.  The open flight of stairs was five risers high with a tread that measured approximately 60 inches in length.  Table D-2 required that both ends of the tread be equipped with a stiar rail or the equivalent.  This violation was observed on 09/11/18.
