29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about September 4, 2018, at a work-site located next to 112 Ridgeview Dr, Seven Hills, Ohio 44131; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 9.4 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 4, 2018, during the installation of roofing material at a job site located at 112 Ridgeview Dr, Seven Hills, Ohio 44131; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 9.4 feet by utilizing a fall protection system.
