29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. On or about 9/7/2018, at 9465 Miami Valley Road, Fort Valley, GA: Employees working on the second level of the building were not protected from a fall hazard at or above 10 feet from ground level. The employer failed to install a complete guardrail system on the 10 window openings on the upper level of the building.

The employer failed to install a midtrial at the upper level surface access area where the job-made wooden ladder was installed.
