29 CFR 1910.29(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds applied in a downward or outward direction within 2 inches of the top edge, at any point along the top rail.  (a) Ranger District Warehouse - On September 10, 2018, employees were exposed to falls of approximately 10' while working around the scissor gates at the forklift loading areas of the second story.  The scissor gate deflected by at least 6 inches with only slight pressure.

29 CFR 1910.29(f)(1)(iii)(A):  The employer did not ensure that the top rail of a stair rail system also serving as a handrail system was not less than 36 inches (91 cm) and not more than 38 inches (97 cm) as measured at the leading edge of the stair tread to the top surface of the top rail.  (a) Flatcreek Work Center - Road Maintenance Shop - On September 11, 2018, employees were exposed to a fall of approximately 9' when transiting the staircase up to the mezzanine with a top handrail measuring 28".

(a) Ranger District Warehouse - On September 11, 2018, employees were exposed to contact with an exploding grinding disc/wheel on the Bosch 6" bench grinder with a work rest that was folded completely away from the grinding surface.
