29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process:    1) On September 10, 2018, and at prior times, the process hazard analysis for the AA Automation did not consider the hazard of an anhydrous ammonia release resulting from the automated valve for the vaporizer oil blow down being set or stuck in the open position.  An anhydrous ammonia release occurred in July 2018 when the automated valve was manually left in the open position.

29 CFR 1910.119(d)(3)(i)(B): Process safety information pertaining to the equipment in the process did not include the piping and instrument diagrams (P&ID's):       1) On September 12, 2018, and at prior times, the P&ID was inaccurate or did not represent existing equipment that was a part of the process in the following instances:    a) an automated valve for the vaporizer oil blow down was not listed on the P&ID.  The valve did not have an identification tag in the field.      b) valve AAS-4, which had been removed, was still listed on the P&ID.
