29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about August 13, 2018 and times prior; Employee exposure to falls: The employer failed to ensure an employee installing vent covers on the top of a travel trailer was protected from falls which exposed him to falls of approximately 8 feet 10 inches to the concrete floor inside the new building.  b) On or about August 01, 2018 and times prior; Employee exposure to falls: The employer failed to ensure an employee sealing on the top of a trailer was protected from falls due to using inadequate fall protection (too much slack in life line) which exposed him to falls of approximately 9 feet to the dirt/gravel/concrete pad of the outside fall protection area.  c) On or about August 23, 2018 and times prior; Employee exposure to falls: The employer failed to ensure an employee replacing rotten wood on the top of a trailer was protected from falls due to using inadequate fall protection (too much slack in life line) which exposed him to falls of approximately 9 feet to the dirt/gravel/concrete pad of the outside fall protection area.  d) On or about July 10, 2018 and times prior; Employee exposure to falls: The employer failed to ensure an employee working on an air conditioner on the top of a trailer was protected from falls due to using inadequate fall protection (too much slack in life line) which exposed him to falls of approximately 9 feet 6 inches to the dirt/gravel/concrete pad of the outside fall protection area.
