29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    (a) 700 -800 Breezy Heights Way Lititz, PA - Employees utilizing a JLG 600-S aerial lift to access the 3rd floor balcony (approximately 26 feet), were not wearing harnesses on or about September 14, 2018.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  700 -800 Breezy Heights Way Lititz, PA  - There was no guard rail an no fall protection system was implemented for the balcony of the third floor (approximately 26 feet) where employees worked, on or about September 14, 2018.      Abatemen certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
