29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:    On or about September 12, 2018 the employer was performing horizontal bore drilling to replace a service gas line without  protecting the old underground gas service line and prevent damage to the line that exposed employees to fire, explosion and inhalation hazards.
