29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. On or about September 17, 2018 and at times prior thereto, the employer did not ensure that the JLG telehandler was being operated according to the Safety Standards for Powered Industrial Trucks: a. The load engaging means was not fully lowered when the operator of the powered industrial truck dismounted the cab and was more than 25 feet away. This exposed employees to struck-by and crushed-by hazards. b. The trash hopper was not secured to the powered industrial truck. This exposed employees to struck-by and crushed-by hazards.
