29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) 151 Du Rhu Drive (Building 2 - Southeast Exterior Wall Area): On or about September 17, 2018, the employer failed to instruct or train their employees in recognizing, eliminating or controlling hazards associated with work performed in a JLG 800SJ Boom lift thus exposing their employees to hazards such as but not limited to catapult hazards of up to approximately fifty five (55) feet to the concrete or ground below.  b) 151 Du Rhu Drive (Building 2 - Southwest Exterior Wall Area): On or about September 17, 2018, the employer failed to instruct or train their employees in recognizing, eliminating or controlling hazards associated with work performed in a JLG 660SJ Boom lift thus exposing their employees to hazards such as but not limited to catapult hazards of up to approximately fifty five (55) feet to the concrete or ground below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) 151 Du Rhu Drive (Building 2 - Southeast Exterior Wall Area): On or about September 17, 2018, the employer failed to ensure employees were using a body belt with lanyard attached to the approved anchor point while using an aerial lift (Model: JLG 800SJ) to perform painting activities; exposing employees to catapult hazards and fall hazards of up to approximately fifty five (55) feet to the concrete or ground below.  b) 151 Du Rhu Drive (Building 2 - Southwest Exterior Wall Area): On or about September 17, 2018, the employer failed to ensure employees were using a body belt with lanyard attached to the approved anchor point while using an aerial lift(Model: JLG 660SJ) to perform painting activities; exposing employees to catapult hazards and fall hazards of up to approximately fifty five (55) feet to the concrete or ground below.
