Holiday Lodge, Oak Hill WV:  On or about 09/19/18, employees working near the edge of the roof while hoisting a new HVAC onto the roof were not protected from falling approximately 20' to the ground below.

Holiday Lodge, Oak Hill WV:  On or about 09/19/18, employees working on a roof to install a new air handler had not been trained in the hazards associated with working from heights.
