29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 2856 Conway Wallrose Road, Baden, PA: On or about September 19, 2018, during roofing activities, the employer did not ensure that an appropriate system was in use at the job site to identify and correct unsafe working conditions. Deficiencies noted during the inspection included, but were not limited to, fall hazards and ladder safety hazards, both of which exposed employees to falls of up to at least approximately 20 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 2856 Conway Wallrose Road, Baden, PA: On or about September 19, 2018, during roofing activities, the employer did not ensure that a ladder used to gain access to/egress from the roof was properly set up in that it was only extended approximately 18 inches above the roofline, exposing employees to fall hazards of up to at least approximately 20 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) Job site, 2856 Conway Wallrose Road, Baden, PA: On or about September 19, 2018, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices were in use where employees were exposed to fall hazards of up to at least approximately 20 feet.
