29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented:                                                                                                                                The employer is failing to ensure that employees are protected from fall hazards. This was most recently documented at 21809 Marinda St., Elkhorn, Nebraska 68022. An employee was wearing a harness without attachment to a lanyard with an anchor point while working on the roof of a home which was approximately 10 feet from a lower level.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.

This was most recently documented at 21809 Marinda St., Elkhorn, Nebraska 68022 on September 17, 2018. The employer did not have a training program to ensure that an employee minimized hazards when exposed to fall hazards.  The employer did not ensure that an employee knew to use adequate fall protection when they were performing residential roofing work on a house.    Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.

This was most recently documented at 21809 Marinda St., Elkhorn, Nebraska 68022 on September 17, 2018.  The portable ladder being used by an employee to access the roof was being used at an angle of approximately one-half the working length of the ladder (10 feet high and 5 feet distance between the foot and top support).    Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.
