This was most recently documented at 21809 Marinda St., Elkhorn, Nebraska 68022. An employee was wearing a harness without attachment to a lanyard with an anchor point while working on the roof of a home which was approximately 10 feet from a lower level. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed.
