29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a)   On or about September 21, 2018 at the jobsite located at 1812 Amazon Drive, Plano, TX, at least one (1) employee was working from the ground passing pieces of plywood to the employer at the top level.  The employee did not have on a hard hat and was exposed to head injuries where overhead hazard exists.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    (a)   On or about September 21, 2018 at the jobsite located at 1812 Amazon Drive, Plano, TX, at least one (1) employee was working from the ground and cutting plywood with a Makita saw 5007F.  The employee was not wearing safety glasses and were exposed to eye injuries.
