Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     On or about September 19, 2018, An employee was exposed to a fall hazard of approximately 12 feet on a construction site in Urbana, Illinois, and fall protection was not being used on a low slope roof.

There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
