29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about September 20, 2018, the employer failed to report the hospitalization and amputation injury of an employee who sustained multiple crushing injuries after being knocked off the side of a flatbed trailer, and struck by the metal shafts/billets that rolled in his direction. The incident which occurred on September 10, 2018, was not reported until September 13, 2018.
