29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects:  (a)  On or about 09/24/18,  at the Iglesia Evangelica del Nazareno, located on 99 Stoinewood Avenue, in Rochester, New York, employees cleaning roof debris and materials were not protected against head injuries from falling roof materials and tools while working under co-workers removing roof materials.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about 09/24/18, during the installation of roofing shingles, at the Iglesia Evangelica del Nazareno, located at 99 Stonewood Avenue, in Rochester, New York,  employees were exposed to falling 17 feet to ground level, as a result of not wearing or having conventional fall protection.    NO ABATEMENT CERTIFICATION REQUIRED
