29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Cleaning area - On or about September 14 2018 and at times prior to, employees engaged in cleaning and sanitizing cooking and backing material were using corrosive chemical products including but not limited to; F-29, F-184, F-110, Evnviroguard Sanitizer, Evnviroguard drain treatment, Perafoam and Power Foam ALS with no emergency eye wash station within the perimeter in accordance with manufacturer's instruction.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about September 24, 2018, and at times prior to, employees to hazards associated with improperly stored fire extinguishers where the following deficiencies found included, but were not limited to: a) Docking station - Access to the fire extinguisher was obstructed by empty bread racks that were placed in front of it and it was also not marked. b) Bread storage area - Access to the fire extinguisher was obstructed by racks filled with bread and it was also not marked. c) Cleaning area - Access to the fire extinguisher was obstructed by buckets, a mop and it was also not marked.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Throughout the facility: On or about September 24, 2018, and at times prior to, employees used the Toyota sit-down type LP-gas forklift, model TO25C, SN #562412A with a personnel platform to lift employees to perform maintenance tasks. Employees were exposed to falling from distances up to 6-feet to the concrete below. The manufacturer did not approve the use of the platform.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, moving parts: a) Circular conveyor - On or about September 24, 2018, and at times prior to, the employer did not ensure that all running nip points and rotating parts of the machine were guarded. The metal conveyor that moves bread to be cool from the oven was not guarded where employees perform tasks such as, but not limited to, loading bread and walking near and around the equipment. Employees were exposed to caught-in between hazards. b) Oliver Machine Model NO. 787G , SN: 108132 - On or about September, 24 2018, and at times prior to, the employer did not ensure that moving blades of the machine were guarded. Employees placing to be cut and picking up bread on the machine was not guarded, exposing them to caught-in between hazards.
