29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The controlling contractor is failing to ensure that all employees of the sub-contractor JA Roofing are protected from fall hazards while working on a residential roof.  This most recently occurred on September 26, 2018 on the jobsite located at 2203 S. 184th Cr, Omaha, NE 68130.  Six employees from JA Roofing were observed working on the roof of a single-story house where there is an eight in twelve steep sloped pitch at heights greater than six feet above the ground.
