29 CFR 1926.502(d)(16):  Personal fall arrest systems, when stopping a fall, shall be rigged such that employees can neither free fall more than 6 feet, nor contact any lower level:       (a)   MH Construction at 3200 Stanford Rd., Fort Collins, CO 80525:  On and before September 25, 2018, the employer did not ensure that the personal fall arrest systems used by employees limited the fall distance to 6 feet, nor prevented contact with the lower level.  Two employees engaged in framing activities on 2nd and 3rd level exterior decks were wearing fall harnesses attached to anchored vertical lifelines; however, their rope grabs and lifelines were not properly adjusted so that their fall protection systems would provide adequate fall protection. Conditions resulted in employees exposed to fall hazards of greater than 6 feet, and with high potential of contacting the ground below at approximate distances of 9 and 18 feet.
