29 CFR 1910.119(d)(3)(i)(B):  The employer did not include all equipment that was part of the process on piping and instrument diagrams:  Worksite, 436 Doremus Avenue, Newark, NJ  a) The employer's piping and instrument diagram  (P&ID)  T_NWR2_P030010 for Pentane Tank T-101 did not accurately represent existing equipment that was part of the process in that the P&ID was missing the following information:  1- There were two 1/2" drains present in the field on the pentane vapor piping that were not on the P&ID.  2 - There was a 1/2" High Point Vent (HPV) present in the field on the pentane vapor piping that was not on the P&ID.  3 - There was a control valve labeled on the 2" nitrogen supply line on the P&ID that was not present in the field, instead it was a check valve.  4 - There was a gate valve with a hand wheel labeled on the P&ID on the 2" nitrogen supply line, which was actually a ball valve with a hand wheel.   5 - Field instrument pressure level  transmitter  PT14 1230 and associated control devices were labeled on P&ID but was not present in the field on the 2" nitrogen supply line.  6 - Field instrument temperature level transmitter LT14 1304 and associated control devices was labeled on P&ID but was not present in the field on the 2" nitrogen supply line.  b) The employer's P&ID T _NWR2_P030002 for Pentane Tank T-101 did not accurately represent existing equipment that was part of the process in that the P&ID was missing the following information:  1 - RV 194 set at 25 psi was present in the field on the 2" pentane vapor balance pipe, however it was not shown on the  P&ID .   2 - RV 195 was present in the field on the 2" pentane vapor balance pipe and the P&ID, but the psi was not lableled.  3 - RV 193 and its pressure 25 psi  were present in the field on the 3" pentane liquid pipe, however the P&ID was not labeled with its identification RV 193.  4 -  RV 192 and its pressure 25 psi  were present in the field on the 3" pentane liquid pipe, however the P &ID was not labeled with its identification RV 192.  Violation occurred on or about 9-26-18
