29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  (a) This violation occurred on or about September 25, 2018, at a job site located at 1453 Coit Road, Plano, TX; where employees were exposed to fall hazards while working from a Genie aerial lift TM TZ-50, 469293, serial number TZ5012 336 and installing LED flood lights on light poles.

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started:  (a) This violation occurred on or about September 25, 2018, at a job site located at 1453 Coit Road, Plano, TX; where employees working from the Genie aerial lift TM TZ-50, 469293, serial number TZ5012 and installing LED lights to the light pole were exposed to an electrical hazard.
