29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about September 26, 2018 an employee installing shingles on a 10:12 pitch two story house was not protected from the 22.5 ft. fall hazard due to an incomplete fall protection system utilized.

29 CFR 1926.503(a)(2)(iii): The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used;  On or about September 26, 2018 the employer did not ensure an employee potential exposed to a 22.5 ft. fall hazard was trained on adjusting the fall restraint system to ensure he was connected at all times while on the elevated work surface.
