29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Employees were not trained on the use of the Globally Harmonized System (GHS) of Chemical Classification and the associated pictograms, signal words and other warnings found on labels and Safety Data Sheets (SDS) to inform employees of chemical hazards. (b) Employees that used paints containing an isocyanate component, were not trained on the hazards of isocyantes, that include skin irritation and sensitization.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: (a) A half face elastomeric respirator given to the painter(s) for their voluntary usage was not stored in a sealed container. The respirators were left in the open and not protected from contamination, exposing them to contamination from paint over spray, solvent vapors and welding fume.
