29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    (a)    Mill:  On or about September 26, 2018 a Patton Floor fan (Model 30 ACH, Serial number 94-309-CO3824) located next to a set of stairs adjacent to the rooftop chain conveyor had a six inch diameter round hole in the middle of the guard over the fan blades.
