Spray Foam Solutions Going Green, LLC, Augusta, GA- At the jobsite, on or about 09/25/18, and times prior to, employees were spraying polyurethane foam containing 4,4-methylenediphenyl diisocyante (Part A); and dichloroethylene, triethyl phosphate, and pentafluorpropan (Part B) to insulate buildings, attics, and homes. Damaged tyvex suits were used while spraying the isocyanates.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Also, other employees were required to go inside the spraying area to assist with the hoses, filling the pot, cleaning, etc.  The personal protective equipment (PPE)  assessment had not been performed and was not in writing.
