On or about 9/27/2018 at 2820 Ogden Trail, Lot 101, Sardis Falls subdivision, Buford, GA where the employer failed to maintain the wire rope on the Tranzporter material elevator, installing a mesh rock climbing rope in its place exposing workers on the ground to potential struck-by hazards.
