29 CFR 1926.304(d): 29 CFREach portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw. On or about September 27, 2018: At the worksite located at 411 Dixon Drive in Gainesville, Georgia an employee cutting rafters for a new residential structure was exposed to amputation hazards when the employer failed to ensure that the guard of the saw was operating correctly.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about September 27, 2018: At the worksite located at 411 Dixon Drive in Gainesville, Georgia an employee utilizing a job built scaffold system was exposed to fall hazard of approximately 7 feet when the employer failed to ensure that each platform on the scaffold system was at least 18 inches wide.
