29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: At the residential construction located at 208 Elissa Drive, Raymore, Missouri: Employee was exposed to struck-by hazards. Employee engaged in framing activities, using a pneumatic nail gun, was exposed to struck-by hazards in that eye protection was not provided.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the residential construction site located at 208 Elissa Drive, Raymore Missouri: Employees were exposed to fall hazards. Employees engaged in framing activities were nailing boards. Employees were exposed to fall distances of approximately 10 feet without the use of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the residential construction located at 208 Elissa Drive in Raymore, Missouri: Employees were exposed to fall hazards by using a ladder which did not extend 3 feet above the landing surface of the scaffold to which they gained access. The fall hazard is up to 10 feet.
