29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  On September 25, 2018, at a jobsite located at 7140 Wornall Road, Kansas City, Missouri, an employee was exposed to fall and caught in hazards when a ladder used to access the roof was placed on a fire escape adjacent to a doorway and was not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear:  On September 25, 2018, at a jobsite located at 7140 Wornall Road, Kansas City, Missouri, an employee was exposed to fall and caught in hazards when a ladder used to access the roof was placed on a fire escape adjacent to a doorway so that the area around the bottom of the ladder was not kept clear.
