Or, on roofs 50-feet or less in width, each employee was not protected by use of a monitoring system.    (a) Tortillas Mexico Mexico Inc. at 840 West 11th Avenue Denver, CO 80204: : On or before October 1, 2018 the employer did not ensure that employees were protected from fall hazards while performing roofing work on a commercial building.  Employees were performing work at the edge of a flat roof approximately 20 feet above the ground without the use of fall protection.  Flagging barriers were not installed 6 feet or more away from the edge of the roof. This condition exposed employees to fall hazards of approximately 20 feet.
