29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)Demmer Investments IV, Inc. dba Quality Tool, Inc. at 301 Sunrise Ridge Road, Brookings, SD 57006: On or about and at times prior to October 4, 2018, the employer did not ensure employees were protected from amputation hazards in that, the Marvel Spartan horizontal band saw, model number PA13/3, was operated without the guard in place covering the unused portion of the blade. This condition exposed an employee to amputation hazards.
