29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On 27 September 2018, an employee lost their balance and fell on their face while at work, requiring hospitalization for 48 hours. The hospitalization was not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration. The Austin Area Office was notified on 3 October 2018 at approximately 3:34 PM.
