29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.      On the south side of the construction site, employees were not wearing hardhats while working below other employees that were engaged in cornice activities from a jack scaffold.  The employees were exposed to struck-by hazards from falling objects.

On the south side of the construction site, employees engaged in cornice activities used pneumatic nail guns and were not wearing eye and face protection.  The employees were exposed to struck-by and eye injuries.

On the south side of the construction site, employees engaged in cornice activities utilized an extension cord that was missing the ground pin to operate a Makita skill saw exposing them to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.        On the south side of the construction site, employees using an extension cord with the outer sheathing pulled away exposing the internal wiring were exposed to electrical shock hazards.
