29 CFR 1910.23(c)(7): Ladders placed in locations such as passageways, doorways, or driveways where they can be displaced by other activities or traffic were not secured to prevent displacement or guarded by a temporary barricade, such as a row of traffic cones or caution tape, to keep activities away from the ladder: On or about October 9, 2018, at the east roof edge of a Safelite Auto Glass facility located at 182 SW Port Saint Lucie Boulevard in Port Saint Lucie, Florida, employees performing gutter work were exposed to a fall hazard of approximately 14 feet when using portable ladders that were set up on front of open cartport doorways where they could be displaced by vehicle traffic.

29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface did not have side rails that extended at least 3 feet (0.9 m) above the upper landing surface: On or about October 9, 2018, at the east roof edge of a Safelite Auto Glass facility located at 182 SW Port Saint Lucie Boulevard in Port Saint Lucie, Florida, an employee performing gutter work was exposed to a fall hazard of approximately 14 feet when gaining access to a roof by using a portable ladder that did not extend at least 3 feet past the roof's edge.
