29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.     On or about October 9, 2018, during the installation of roofing material at a job site located at 710 Lincoln Way East, Massillon, Ohio 44646; the employer did not ensure employees working on the roof of a commercial location were protected from a fall of approximately 10.5 feet by utilizing a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about October 9, 2018 at the job site located at 710 Lincoln Way East, Massillon, Ohio 44646; during the installation of roofing material on a commercial retail roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.
