29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.      On or about October 9, 2018 the employer allowed employees performing grinding operations to utilize a hand held  grinder without guards to smooth the welds of the structure exposing them to struck by hazards.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket.    On or about October 9, 2018, the employer allowed employees performing grinding operations to stand on the mid rail and top rail of the basket on the aerial lift exposing them to fall hazards.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.   On or about October 9, 2018 employees  were exposed to fall hazards as they worked from an aerial lift at a height of approximately 12 feet with no fall protection to perform  grinding operations during steel erection.
