The employer did not ensure the forklift was inspected and safe to operate prior to use. The forklift seat was severely torn, the brake light cover was missing, and the reverse alarm was inoperable.

YCM, TV Series 2110B, MFG# 0723, Model TVI158B, Vertical Machining Center, Asset #51, doors were not interlocked. 2. YCM 00A MFG# 0001, Model NEX500A, Asset# 61, doors were not interlocked. 3. YCM FX 500A, MFG # 10044, Model # FX380A, Vertical Machining Center, doors were not interlocked. 4. Bridgeport Mill, Asset #M1 was not equipped with a chip guard. 5. Bridgeport Mill, Asset #M5 was not equipped with a chip guard. 6. Green (Unidentifiable) Belt and Disc Sander Top In-Going Nip Points were not guarded. 7.
