29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):    In the warehouse, employees used a Yale #6 forklift to move, unload or load materials. The employer did not ensure the forklift was inspected and safe to operate prior to use. The forklift seat was severely torn, the brake light cover was missing, and the reverse alarm was inoperable.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    In the milling and lathe departments, the employer did not ensure guarding on the Mills, CNC Mills, CNC Lathes, Belt and Disc Sander, and Bandsaws protected the employees from hazards created by point of operation, ingoing nip points, rotating parts, flying parts and chips exposing the employees to struck-by and amputation hazards.     1.     YCM, TV Series 2110B, MFG# 0723, Model TVI158B, Vertical Machining Center, Asset #51, doors were not interlocked.  2.     YCM 00A MFG# 0001, Model NEX500A, Asset# 61, doors were not interlocked.  3.     YCM FX 500A, MFG # 10044, Model # FX380A, Vertical Machining Center, doors were not interlocked.  4.     Bridgeport Mill, Asset #M1 was not equipped with a chip guard.  5.     Bridgeport Mill, Asset #M5 was not equipped with a chip guard.   6.     Green (Unidentifiable) Belt and Disc Sander Top In-Going Nip Points were not guarded.  7.     HAAS VF-2, CNC Vertical Mill, Asset #22, doors were not interlocked.  8.     HASS VF-7, CNC Vertical Mill, Asset #21, doors were not interlocked.  9.     HASS VF-2, CNC Vertical Mill, Asset #19, doors were not interlocked.  10.   HASS VF 4, CNC Vertical Mill, Asset #17, doors were not interlocked.  11.   HASS VF, Asset #3, right side glass was broken, and doors were not interlocked.  12.   HASS Super 2, Asset #4, doors were not interlocked.  13.   HASS VF4 20HP Vector Drive, CNC Vertical Mill, top, right side glass was broken.  14.   HASS VF2, CNC Vertical Mill, Asset #6, doors were not interlocked.  15.   HASS VF2B, CNC Vertical Mill, Serial # 25037, glass was broken, and temp repaired with cardboard.  16.
