In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about October 5, 2018; roof area of new edition; employees were walking the top-plate without fall protection to protect them from falling 30-35 feet to ground level.                           b) On or about October 5, 2018; second floor; employees were working near floor openings such as but not limited to the stair opening that was not guarded, exposing them to a 10-foot fall to the lower level.      c) On or about October 5, 2018; second-floor area; employees were working near unguarded wall openings, exposing them to a 20-foot fall to ground level.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.     a) On or about October 5, 2018; Second-floor area; Employees used a 6-foot step ladder to access the roof/top plate which was 4-feet below the platform, exposing employees to a potential 10-foot fall to the lower wooden floor or 20-feet to ground level.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
