29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or alternatively, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), by the use of a safety monitoring system alone: a) Job site, 1242 Brighton Road, Pittsburgh, PA: On or about October 12, 2018, during roofing work, the employer did not ensure that appropriate fall protection systems/devices were in use where fall hazards of up to approximately 14 feet existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 1242 Brighton Road, Pittsburgh, PA: On or about October 12, 2018, during roofing work, the employer did not ensure that an extension ladder was appropriately used in that it was determined to be extended only approximately 29 inches above the landing surface, exposing employees to a fall hazard of up to approximately 14 feet. Hooper Roofing Inc. was previously cited for a violation of this occupational safety and health standard ,or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1116341, citation number 1, item number 1, and was affirmed as a final order on February 3, 2016, with respect to a workplace located at 2339 East Carson Street, Pittsburgh, PA.
