29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:      a) Job site, 1242 Brighton Road, Pittsburgh, PA: On or about October 12, 2018, during roofing work, the employer did not ensure that an extension ladder was appropriately used in that it was determined to be extended only approximately 29 inches above the landing surface, exposing employees to a fall hazard of up to approximately 14 feet.    Hooper Roofing Inc. was previously cited for a violation of this occupational safety and health standard ,or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1116341, citation number 1, item number 1, and was affirmed as a final order on February 3, 2016, with respect to a workplace located at 2339 East Carson Street, Pittsburgh, PA.
