On or about October 11, 2018, during the installation of masonry brick at the job site located at 7521 Roman Ave NW, Massillon, Oho 44646; the employer did not ensure employees were provided proper access/egress from a fabricated frame scaffold. Employees were observed gaining access to the working surface by climbing the cross braces on a bricklayers scaffold.
