OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts:      a) On or about October 11, 2017 and at times prior, employees were exposed to possible crushing hazards from two Forward vehicle lifts that have loose cables on the locking mechanism. The cables would not release the locking mechanism on one side of the lifts and would cause the vehicle to tilt to its side when lowering down the lifts. The lifts were not properly inspected, taken out of service for repair, and maintained prior to the employees' use.       One feasible and acceptable method to correct this hazardous condition would be to follow the requirements in accordance to the instructions required in the ANSI/ALI ALOIM: 2008 Safety requirements for operation, inspection, and maintenance standards.
