The employees framing a new house were walking along the scaffolding walkway plank that was 5 1/2 inches wide and not the required 18 inches.

The employee framing new house was walking along the wooden walk way plank that was 26 inches away from the house wall that was not guarded to protect employee from falling 5 feet to the ground below.

The employees gained access to the roof using an aluminum extension ladder that was not secured allowing the ladder to fall to the left making it unsafe for use.
