29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), and (c)(4)(ii)(D):    The employer is failing to provide specific lockout/taqgout procedures, according to the requirements of the standard, for the ACC (steam) autoclave.  The "MONTHLY DOOR GASKET REPLACEMENT 5AU-0004 SAP EQUPMENT 20021492" procedure provided at the time of the inspection, was not a lockout/tagout energy control procedure.  It did not include a specific statement indicating it's use for energy isolation, but instead stated the purpose was for gasket replacement; it did not include specific procedural steps for shutting down and isolating to control for hazardous energy, it did not include procedural steps for energy isolation for steam, and it did not include steps for testing for energy isolation and verifying the effectiveness of energy isolation.    Abatement documentation is required for this violation.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    The employer is failing to inspect the lockout/tagout program annually.  An inspection had not been performed for over 17 months.  At the time of the initial opening of the inspection, on 10-16-18, the most recent lockout/tagout inspection was dated 4-21-17 ("2016 Lock-out/Tag-out Audit (St. Louis Facilities)".      Abatement documentation is required for this violation.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    The employer is failing to ensure that a lock is placed on an energy isolation device by all authorized employees performing work.  During maintenance work on the ACC Autoclave, between 10-1-18 and 10-9-18, by three different maintenance employees, who were authorized to perform lockout/tagout work, only one of the three placed a lock on the autoclave isolation device that was left on the equipment during work by all three employees.    Abatement documentation is required for this violation.
