Employees were working on the roof of a house without: (a) wearing harnesses, (b) being connected to any lanyard or ropes, and (C) using anchors attached to the roof itself. Employees were exposed to falls of over 7' 4" to the concrete driveway below.  The roof pitches of this structure were greater than 4 in 12. Specifically, the roof pitch of the upper level was approximately 12 in 12 and the roof pitch of the lower level was approximately 4 in 12. Therefore, all roofers are required to use fall protection equipment while working on those levels.

The owner and seven employees were using two twenty-eight foot extension ladders (one aluminum and one fiberglass) to enter and exit the roof. While the aluminum one was properly set up, the fiberglass ladder did not have the side rails extended to at least three feet above the landing surface on the edge of the lower level of the roof. Ladder was actually extended to two feet above the eve only.
